This paper helps friends and family work together to keep children feel safe, cared for, and understood.

Daily Routines and Feeling Good

Time/Date

How do they get dressed or bathe?
When do they sleep and wake up?
Have they been hurt or had an accident recently?
What medicine have they taken recently?
Were there unusual or important events recently?

Food and Meals

Time/Date
Where food was bought?
What food was bought?
If something had to be changed or replaced?
Is there anything to know about meals or snacks?

Health and Doctor Visits

Time/Date
What is the doctor or nurse’s name?
Which adult was there?
Where did the visit happened?
Why did the visit happen or what was noticed?

Examples.
The child felt sick in the car
The child had trouble talking or sharing feelings

School and Learning
Time/Date

We're there any school events?

We're there any changes in behavior?
Is there anything that helps the child learn or feel better at school?

Ouings and Activities
If a trip takes more than one hour total, please write it down ahead of time. This helps everyone plan and make sure the child is cared for.

No hitting or hurting at all
Use kind and respectful words
Tell the truth about money spent
Tell others if there is an accident
Tell others if they stay somewhere overnight
Don't talk about weight or dieting around the child
Respect all people and cultures

Everything written should be calm, honest, and about helping the child. The goal is safety, fairness, and care.
