Formal Complaint Example
I am writing to provide an update and to formally submit a complaint regarding my ongoing involvement with CPS.
Previously, I was informed that while my case is in appeal status, your office would not be able to assist. However, I would like to bring forward developments that I believe are important for your office to be aware of.
Despite the appeal status, CPS continues to be directly involved in my case through ongoing, in-person visitation, which has been taking place for several months.
I also want to raise a concern regarding service consistency. Last year, while publicly reported statements were made indicating that visitation and service obligations were being met, I personally experienced extended periods where my visits were not being scheduled. This created a significant gap between what was being reported and what I was directly experiencing in my case.
Additionally, I formally requested a complete copy of my CPS case file on October 16, 2025. I was later informed that my records were ready for release. However, I have not received the records by certified mail as requested, and I have made multiple unsuccessful attempts to obtain them in person. Communication from the assigned worker has been inconsistent, and follow-up commitments have not been met.
Given these circumstances, I am requesting that your office review or document this situation as a complaint regarding:
Ongoing CPS involvement during an active appeal
Failure to provide records after confirming they are ready
Inconsistent scheduling of visitation services
Lack of communication and follow-through by CPS staff
I understand the limitations previously explained to me, but due to the continued direct involvement and the issues outlined above, I believe this matter warrants review or formal documentation.
